Carers Home Page

Do you look after a family member of friend who is ill, frail, disabled or mentally ill? If so you are a carer and we’d like you to tell us so we can help you.

We feel that caring for someone is a crucial role in the community and can be very demanding and isolating for the carer. As a carer you are entitled to have your needs assessed by Adult Care Services and we can arrange this for you.

You can simply tell your GP the next time you see them that you are a carer or fill the following form. Carer Form

This will be recorded in your medical notes and allow us to continually offer you support. It doesn't matter if the person you care for is not registered at Crown Heights Medical Centre.

NHS choices has a wealth of information on being a carer and we have included some links to topics which you may find particularly useful.
